The FullWell Community Portal has been discontinued

In order to stay abreast of population health tools industry standards and the expectations of our member practices, FullWell and CHN are replacing the community portal with a new tool.  

The new portal will have all the functionality of the old portal as well as some enhanced functions. During the transition there will be some temporary work flows that CHN and FullWell will use to support the practice during the transition. The current FullWell Community Portal was discontinued on June 30, 2017.

  • FullWell is in the planning stages with our new portal partner - Optum.
  • All the current functions of the present portal will be included in the new tool.
  • All of the data and attestations that practices have entered in the present portal will migrate over to the new system.
  • The new solution will include the functionality of the current portal along with additional enhancements.
  • Development of the new portal is underway with an anticipated launch date in early fall, 2017.
  • The switch to the new portal will include a transition period between June 30 and when the new portal goes live (anticipated early Fall 2017). We commit to communicating and training you on any interim processes as needed.
  • Members of the CHN performance improvement team and FullWell portal support team are ready to assist you in the interim processes while the new portal is developed.
  • Extensive training on the new portal product will be offered prior to go-live. As with the previous portal launch we will offer webinars, face-to-face and hands-on training.
